Employee Responsibilities
All employees are required to:

4. Understand, follow and support the policy and procedures adopted by the governing body
of this facility.
5. Communicate any ideas, problems or concerns timely to the appropriate person.
6. Refrain from idle gossip.
7. No administrator, manager or staff person shall be under the influence of alcohol or other
controlled substances while at work for this agency.

Local and Federal Criminal Record / Fingerprint Check

Resume (showing at least five years of experience in human service or closely related field.

Credentials, (including diploma or equivalent)

Proof of Insurance

Valid Driver’s License

Motor Vehicle Report

Physical Exam /TB Skin test

CPR & 1st aid Cards (not applicable to RNs)

At least 16 qualified training hours that is pertaining to their field of work. (Licensed RN are to maintain 8 hours of continued education.
